Registered Nurse – Case Manager

This is a Contract position in Ottawa, ON posted May 14, 2021.

Your Opportunity: Integrated Home Care uses a strengths-based Client Centered Case Management practice framework.

Using a person-centered approach you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care.

In collaboration with the client/family and interdisciplinary team you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization.

You will report to the Home Care Manager and will be responsible for coordinating and providing quality care to clients in the community.

You will provide nursing assessments, case coordination and direct care in partnership with clients and their families in their own residence or an alternate care setting, as determined by the assessment.

The successful incumbent will be expected to demonstrate proficiency in the following areas: personal and professional accountability, systems thinking, creative thinking, communicating and building relationships, organizational and time management skills, interpersonal skills and intermediate level of computer skills.

Description: As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being.

In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families.

You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance.

You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Classification: Registered Nurse Union: United Nurses of Alberta Department: CC ADULTS HC Primary Location: East Calgary Health Centre Multi-Site: At or Out of (UNA only) FTE: 1.00 Posting End Date: 25-JAN-2021 Temporary Employee Class: Temp F/T Benefits Date Available: 15-FEB-2021 Temporary End Date: 15-FEB-2022 Hours per Shift: 7.75 Length of Shift in weeks: 8 Shifts per cycle: 38 Shift Pattern: Days, Weekends Days Off: As Per Rotation Minimum Salary: $36.86 Maximum Salary: $48.37 Vehicle Requirement: Driver’s License, Vehicle Required Required Qualifications: Completion of an accredited nursing education program.

Active or eligible for registration and practice permit with the College and Association of Registered Nurses of Alberta (CARNA).

Current Basic Cardiac Life Support
– Health Care Provider (BCLS-HCP).

Additional Required Qualifications: This role requires travel; possession of a valid drivers’ license and access to a motor vehicle in good repair for work related purposes is a requirement of this position.

You will be required to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job.

For example: lifting of approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common within this job.

Learning resources and training are provided to safely perform work.

Preferred Qualifications: Bachelor’s degree in Nursing.

Experience working with acute clients and/or long term clients with a focus on chronic disease management.

Recent experience in a community based setting (Home Care experience preferred).

Experience providing case management services.

Canadian Nurses Association (CNA) certification in Gerontology.

Experience in Home Parenteral Therapy (HTP), wound skin, ostomy care, palliative support and client teaching.
