Dual Diagnosis Case Manager

SUMMARY: Valor & Solutions is a bilingual non-profit organization based in Ottawa, which serves the Eastern Region (Prescott-Russell; Stormont, Dundas & Glengarry; Ottawa and Renfrew County). Valor & Solutions offers services to adults diagnosed with an intellectual disability or dual diagnosis who present with challenging behaviors or complex medical needs. Valor & Solutions also offers support, training, consultations, and recommendations to the person’s support system. We equip the person’s support network with the necessary tools or strategies to best support the individual and to increase their quality of life.

The Dual Diagnosis Case Manager is responsible for establishing a professional, supportive and therapeutic rapport with the person receiving services and their support system. The Dual Diagnosis Case Manager will provide supports to persons who have been identified with complex multi-system needs and require a high level of active engagement.

QUALIFICATIONS AND REQUIREMENTS

  • Master’s degree preferred in social work, or related degree; and three (3) to five (5) years of experience of intervention with adults with intellectual disabilities, and/or mental health disorders, and/or challenging behaviours;
  • Advanced knowledge and skills related to developmental disabilities, mental health, family work, and system issues;
  • Demonstrated ability to work as part of an inter-professional team;
  • Bilingual (French and English) essential, oral and written;
  • Excellent interpersonal skills;
  • Ability to intervene and supply valuable and tangible tools and resources;
  • Ability to work independently and demonstrate initiative;
  • Ability to manage time and priorities;
  • Experience in coordinating responsive individualized support to persons with complex needs;
  • Demonstrated skill in establishing empowering and supportive partnerships with persons with complex needs;
  • Good knowledge of the services available in the East region (Prescott-Russell, Cornwall, Ottawa and Renfrew) would be an asset.

MAIN TASKS

  • Provide cross-sector coordination (criminal justice, health care, etc.) that supports case resolution, including service navigation; liaising with cross-sectoral service partners; coordinating referrals and support; and when necessary, advocating for required supports and services.
  • Conduct an evaluation of the needs of the person.
  • Plan for crisis prevention, intervention and support.
  • Develop capacity and resilience in a person’s network and coordinate supports from a range of sources.
  • Lead, develop, facilitate, coordinate, monitor and update support plans.
  • Help with the navigation and collaboration of complex systems to coordinate access to services.
  • Provide person-centered support based on developing skills, competencies and resources to promote the person’s wellbeing and development.
  • Educate, train, and coach caregivers and supports.
  • Offer support in leading certain groups (as needed).
  • Participate in meetings and committees (as needed).
  • Complete necessary documentation according to the agency’s guidelines.
  • Follow administrative and clinical requirements as described in the policies and procedures.
  • Collaborate with other case management supports and community based agencies to ensure efficient use of resources in complex support coordination.

ANNUAL SALARY: $55,704 to 72,095$ (in regards with professional experience, skills and education)
